Entry Form 2 part

Agricultural Society Entry Forms are fully customizable and a digital proof is shown prior to printing. For a nominal fee, we can set the form up for you ($18 non refundable set up fee) or we can print from your pdf. Size is 8.5 x 11 and is printed in black ink on 1 side on 2 part carbonless (white/canary sequence) heavy weight paper (20lb bond equivalent). Each set is padded at the top using a special adhesive and is packaged in 100's. This product is sold by the 100, for larger quantities please request a quote as better per unit pricing will be provided.
